• Your doctor will submit your claim to UMR.
• You’ll then receive an Explanation of Benefits (EOB) from UMR that shows the discounted charges and what you will be responsible to pay.
• UMR then provides this information to your doctor and you will be billed.
• You may choose to use HSA or FSA dollars to pay the bill or pay out-of-pocket.

• At the point of sale, you will pay the CVS Caremark negotiated price of the drug, which will apply towards your annual deductible
• Once you’ve reached your deductible, you will pay a smaller portion of the cost (coinsurance percentage), which will continue until you reach your out-of-pocket maximum.
• Prescription drug claims are eligible expenses to be paid with either an HSA or FSA, if you choose to enroll in either of these benefits.

2. Follow the steps to create an account.
3. Once your account is set up, you can request a visit with a provider at any time. You can select phone, video or app consultations.
4. After your visit, the provider will offer a treatment plan, which may include prescriptions sent directly to your pharmacy.
